Whoops.

I recently was reviewing a patient’s electronic medical record and stumbled across the following note:

Impression: The patient’s primary problem is his wife.

Plan: …

An addendum was added a few minutes later:

I did not mean what I typed above. The impression should read: “The patient’s primary problem is [issue] and his wife is helping with…”

Sometimes, we all hit the “submit” button a little too quickly.
